How to get your OneTouch Ping.

Everything you need to get started is right here and easy to download. The Assignment of Benefits form (AOB) gives Animas permission to contact your insurance company and physician on your behalf. The Patient Information form provides us with the information we need if you want to proceed with obtaining an insulin pump.

Once the paperwork is processed, we will ship the pump and contact you to help ensure that you receive training to get started. Soon after that, you’ll be on your way to a new, freer, more flexible life.

1. Download the AOB and Patient Information forms and print a copy of each.
2. Complete and sign the AOB.
3. Complete the Patient Information form.
4. Fax both forms to 1-877-331-7300. Or mail to: Animas Corporation, 200 Lawrence Drive,
