Take the first steps to better performance and more flexibility

We’re glad you’re interested in the OneTouch^® Ping™. Whether you're ready to begin the process of getting your insulin pump now or just want to familiarize yourself with the steps, you'll find what you need to know here.

There are four steps you'll need to take right now:

1.Download the forms and print a copy of each.

Get the Assignment of Benefits form (AOB) and Patient Information form

You'll get both forms in one file, along with instructions.

You'll need Adobe^® Reader^® to view these files.

2.Fill out the Assignment of Benefits (AOB) form.

This form gives Animas permission to contact your insurance company, contact your physician, and help you throughout the insurance process. A pump specialist determines your benefits, then acts on your behalf to gain insurance authorization. Filling out the AOB does not obligate you to purchase a pump in any way.

3.Fill out the Patient Information form.

This form provides us with the information we need if you want to proceed with obtaining an insulin pump.

4.Fax or mail your forms

Once the paperwork is processed, we'll ship your pump and contact you to help you get started with training.
